Carers

Health & Beyond is named not only to interact with health issues but what is beyond those issues but also as a service provider consider that carers cannot be forgotten in the greater scheme of providing appropriate health and social care to them and not only the ones they care for.

We want to ensure that as a carer you are identified, considered and supported in the job you do. Can we ask you if you care for a relative or friend please inform one of our receptionists, nurse or doctor, and complete a Carer’s Identification Form, which we have readily available at all our localities or on-line which can be found at the foot of this document.

This allows us to keep a register of carers so that we can provide help and advice not only to the person they care for but also you. You may not be a carer of a patient registered within one of our localities but if you are one, identify yourself please because you are one of our patients.
