What is a Patient Participation Group 'PPG'? 

It is a group of patients who volunteer to discuss topics of mutual interest and make positive suggestions for our organisation to consider. 

What is the purpose of me joining the PPG? 

Joining the PPG enables our patients to have their say in relation to the healthcare services we provide to our patients. Your opinions matter to us and are valuable to us. 

We would like you to share your views about our services to help us understand our patient's perspective which will enable us to improve of healthcare services.

When are where do we meet? 

Dates and times of the PPG meetings are listed below. Our PPG meetings are "user friendly" and are fully supported by Health and Beyond.

What do we do? 

We take an active interest in healthcare services and have a say in the way services are planned, developed and delivered. 

Will the questions be medical or personal? 

Questions are asked in relation to the organisation and the services we provide.

During the PPG meetings patients personal health needs are not discussed. 

Members will be able to attend any of the meetings at which ever sites they choose, see below for some of the coming venues and times

Appointments

Important Information - All in and out bound telephone calls are now recorded across all our sites

APPOINTMENTS

Appointments can be made over the phone or in person at our reception desk.

Many of our patients will book to see a doctor or nurse on a regular basis.  However, we do not see enough of some of our registered patients.  If you are between the ages of:

18-74 - we recommend that if you have not seen a doctor or nurse within the last 3 yearsbook a routine appointment.

Over 75 - if you have not seen a doctor or nurse in the last year-book a routine appointment for a health review.

Medication reviews-if you have not had a review of your repeat medications in the last year-book a routine appointment for a review with our Pharmacist.

Pill Checks- if you need a repeat of your Contraception pill you will be asked to make an appointment for a pill check to check you vitals.

Health Reviews-If you suffer from Chronic ailments such as Diabetes, COPD, Asthma, Hypertension, Mental Health Issues or CHF you will be invited in for an annual Health Care Check. 

Routine Appointments

We appreciate that many of our patients work or are in education full time.  Therefore we offer appointments before or after work hours as well as booking of routine appointments up to 8 weeks in advance.  Patients will receive confirmation via text of the booking as well as a reminder close to the day of the appointment.

Urgent on the Day Appointments

For more urgent medical needs that cannot wait for a routine appointment we ask that you ring us between 8 and 9am or come into the surgery on the day to book the appointment.  While we cannot guarantee a specific doctor or time we will do our best to accommodate you on the day.

We have priority on the day appointments for children who are unwell and a full range of nursing services as well.

Online Access

We have access to Routine appointments and repeat prescriptions though our online Patient Access Portal.  Please see reception for set up with an online login and password as well as instructions to help you.

HOME VISITS

We encourage where possible, all our patients to attend the surgery to see a doctor.  Home visits are only for patients who are housebound, terminally ill or frail and there is a medical need.  Transportation issues are not a reason for a home visit.

Please ring the surgery before 11:00am to book same day home visits.  Requests made after this time will be made on the following working day.

Please provide as much information as you can to our reception team and ensure that your contact details are up to date. 

Please note there is no guarantee that a specific doctor will visit you at home and the time is determined by clinical workload and urgency of request.

TELEPHONE APPOINTMENTS

If you need to speak to a doctor or nurse rather than attend an appointment, please ask for a telephone consultation.  These requests are allocated as an appointment within the surgery and therefore are for 10 minutes and for one patient with one query.  Please be aware that many medical concerns cannot be diagnosed over the phone and therefore you may still be required to come in to attend an appointment. If you wish to speak with a doctor or nurse regarding another family member or a patient who is not yourself, we cannot discuss any medical issues without prior written consent from the patient.

EMERGENCY APPOINTMENTS: When we are Closed

If you have a life threatening medical emergency please dial 999.

If you require medical attention when the practice is closed, please phone 111 to access NHS Direct.  This service is run by a team of Doctors, nurses and other health care professionals and will assess and inform you as to the best course of action of medical attention. You will be assessed, given advice and directed straightaway to the local service that can help you best. Calls to NHS 111 are free from landlines and mobile phones.

Walk In Centres

There is an Urgent Care Centre walk-in now located the floor above A&E at New Cross Hospital and Phoenix Health Centre.

Walk-in Centres are for minor health problems, minor injuries and emergency contraception led by experienced, specially trained nurses and are available at:

Open Monday to Friday: 10.00am 7.00pm, Saturday, Sunday and Bank Holidays: 10.00am 4.00pm.

Urgent Care Centre

The Urgent Care Centre (UCC) is situated above the A&E department at New Cross Hospital and is open 24/7, 365 days a year. You can self-present at the UCC reception or you may be directed to it via NHS 111 when appointments will be booked electronically for you. You may also be redirected to this service from A&E if it more appropriate to manage your needs.

Emergencies A&E/Call 999

The local Accident and Emergency Department is New Cross Hospital.Whatever the day or time, if you suspect you may be having a heart attack, stroke, severe acute illness or serious Accident then please dial 999 and you will be advised accordingly. If you are well enough to attend A&E yourself then please do so.

Ask Your Pharmacist

Pharmacists are medicines experts and are able to give free health advice and treatment about common illnesses like colds and flu or sickness and diarrhoea. You do not need an appointment and you can speak to them in confidence.

Enhanced Access Hours - Evening, Weekend and Bank Holiday appointments

Please see Wolverhampton Care Collaborative (WCC) our Primary Care Network website for extended opening hours across the practices for appointments available to you. Appointments are bookable in advance through your Surgery or on the day during the opening times for the extended hours with your surgery.

Prescriptions

Repeat Medication

If you are in need of repeat prescriptions there are several ways for you to request them. 

  1. In person--either mark the items you wish to order on your previous script or fill out one of our convenient script request forms or hand it to our reception staff or place in the script request box.
  2. Online--if you have registered for online access you may order your prescriptions through our online option.

All repeat prescription requests take 72 hours.  Please plan accordingly.

Please be sure to communicate your regular Pharmacy to our reception team if you wish your prescriptions to go through Electronic Prescribing directly to the chemist.

Requests for new medications or medications prescribed by an external provider may require a Medication Review and therefore may take longer to collect.

All controlled drug scripts must be collected in person at the surgery during normal business hours.

Free Prescriptions

Did you know that you can get free prescriptions for any of the following medical conditions?

  • A permanent fistula (for example caecostomy, colostomy, laryng ostomy or ileostomy) requiring continuous surgical dressing or requiring an appliance.
  • Epilepsy for which you need continuous anti-convulsive therapy.
  • Diabetes mellitus and your treatment is not just by diet alone
  • Myxoedema
  • Hypoparathyroidism
  • Diabetes insipidus or other forms of hypopituitarism
  • Forms of hypoadrenalism (including Addisons disease) for which specific substitution therapy is essential
  • Myasthenia gravis
  • A continuing physical disability which means you cannot go out without the help of another person
  • Undergoing treatment for cancer

Ask your GP for an FP29A (Medical Exemption Certificate), also available from the Hospital Pharmacy Department. This will need to be countersigned by your GP, hospital or service doctor.

Test Result

If you have an investigation such as a scan or sample taken (eg: smear or blood test), please contact us any time after 11:00am MondayFriday.

We do not routinely contact patients about their results; it is up to you to contact us. The exception to this is if the results are urgent or require a visit with your doctor to discuss further treatment.

  • Blood test results are usually back within 2 to 4 days
  • Urine and swab results take a minimum of 3 days but may take up to seven days
  • X-rays and other radiological scans make take in excess of two weeks and are dependent upon the hospital workload.

If your test results are back but have not been checked by a doctor, you will be informed and asked to contact us again. Please remember our reception staff are not medically trained and will not be able to interpret your results. Once the results are check by the doctor our reception staff can then let you know the outcome. We cannot give full results over the phone and you may be asked to make an appointment to discuss the result further.

OOH

Extended Opening Hours Evening, Weekend and Bank Holiday appointments

Appointments are bookable in advance through your Surgery or on the day during the opening times for the extended hours with your surgery.

Urgent Medical Assistance

If you require urgent medical assistance you may contact your surgery during the opening hours. If you contact the surgery at other times, you will be given a telephone message with the Out of Hours contact telephone number.

Out of Hours Service

If you require urgent medical assistance, which cannotwait until the surgery re-opens, you should call 111. Calls to the NHS 111service are free from both landlines and mobiles. You will be assessed, givenadvice and directed straightaway to the local service that can help you best.

Your IP address will be sent withyour communication. In rare cases where abuse or criminal activity can be shownto have taken place this may be used by the authorities to trace you.

Confidentiality, Data Protection & Freedom of Information act

We respect your right to privacy and keep all your health information confidential and secure.

It is important that the practice keep accurate and up-to-date records about your health and treatment so that those treating you can give you the best possible advice and care. This information is only available to those involved in your care. 

You have a right to know what information we hold about you. The Freedom of Information Act gives you the right to request information we hold. If you would like to see your records, please contact our Practice Manager. You will be asked to complete a request form and there may be a charge for this information.

Please ask for a listing of all charges for letters, reports and copies of your information.

Information requested by third party organisations outside the NHS will NOT be shared without your explicit written consent.

GDPR Compliance Statement

Introduction

The EU General Data Protection Regulation (GDPR) came into force across the European Union on 25th May 2018 and brings with it the most significant changes to data protection law in two decades. Based on privacy by design and taking a risk-based approach, the GDPR has been designed to meet the requirements of the digital age.

Our Commitment

Health and Beyond is committed to ensuring the security and protection of the personal information that we process, and to provide a compliant and consistent approach to data protection. We have always had a robust and effective data protection programme in place which complies with existing law. However, we recognise our obligations to meet the demands of the GDPR and the UKs Data Protection Bill.

H&B is dedicated to safeguarding personal information and in developing a data protection regime that is effective, fit for purpose and demonstrates an understanding of new regulations.

H&B have a consistent level of data protection and security across our organisation, and are fully compliant with the GDPR from 25th May 2018. Our preparation includes:

  • Information Audits - scheduled network wide information audit to identify and assess what personal information we hold
  • Policies & Procedures - revising data protection policies and procedures to meet the requirements and standards of the GDPR and any relevant data protection laws, including : -
  • Data Protection - our main policy and procedure document for data protection has been overhauled to meet the standards and requirements of the GDPR. Accountability and governance measures are in place to ensure that we understand and adequately disseminate and evidence our obligations and responsibilities.
  • Data Breaches - our breach procedures ensure that we have measures in place to identify, assess, investigate and report any personal data breach within the required timeframe. Our procedures have been disseminated to all employees, making them aware of the reporting lines and steps to follow.
  • Subject Access Request (SAR) - (Requests for information for insurance reports or legal action) we have revised our SAR procedures to meet the 30-day timeframe for responses to third party requestors provided there is appropriate patient consent. Our new procedures detail how to verify the data subject, what steps to take for processing an access request, and what exemptions apply.
  • Privacy Policy - we have revised our Privacy Policy to comply with the GDPR, ensuring that all individuals whose personal information we process are informed of why we need it, how it is used, what their rights are
  • Summary Care Record - Consent or Rescind - Patients are requested upon registration if - they agree to participate in Summary Care Records.  This means that any medically pertinent - information is able to be shared with other health care professionals who are providing care for our patients. Patients can decline to give consent and this will be recorded clearly in their record.
  • Obtaining Consent - we have revised our consent mechanisms for obtaining personal data, ensuring that individuals understand what they are providing, why and how we use it and giving clear, defined ways to consent to us processing their information. We have developed a simple process to withdraw consent at any time.

Any patient over 16 must give written consent for any family member who will need to have access to medical information.  If consent is not given we will be unable to share ay information with parents or care givers.

Universal Credit and Pension Credit for Couples

The introduction of Universal Credit causes some unexpected complications for couples if one is over pension age and the other partner is still below the pension age. This only applies to new claims. Existing payments from Pension Credit made before Universal Credit started in Wolverhampton will continue unaffected even if one partner is still below their own pension age.

What used to happen? Previously, for new Pension Credit claims, both members of a couple could be entitled to Pension Credit from the date when the older member reached their own pension age.

What has changed? Now that Universal Credit is introduced to our area postcodes, both members of a couple must have reached their own pension ages before you can claim Pension Credit. If you try to claim Pension Credit when one of you is below their pension age, you could be directed to claim Universal Credit and the younger partner will be expected to seek work (unless they are sick or a Carer etc.). The usual 6 week delays in first payment of Universal Credit and interruptions to housing benefits etc. will also apply.

What are the alternatives?

  • Where the younger member of a couple is claiming as a Carer AND Income Support is already in payment. It is important to continue with the younger members Income Support claim; there is no need to claim either Pension Credit or Universal Credit. Once the older member reaches pension age then an additional Pensioner Premium will be added to the Income Support amount and that will bring the overall amount which will be equal to Pension Credit rates for a couple. This avoids any interruptions in fortnightly income, housing benefit should also continue without any breaks.
  • If the younger member of the couple is already claiming ESA (income related). Again, there is no need to claim Pension Credit, the ESA claim should continue in the younger persons name and the overall amount should be increased to include a pensioner premium which will bring the total paid to equal the same fortnightly rate as Pension Credit.

We understand that letters are being received by individuals which invite them to claim Pension Credit when they reach their own pension age. These letters wont have taken into account the circumstances of partners etc. But if their partner is younger and fits into one of the above categories then please consider if you fit into one of the above options. If in any doubt, contact the carers team and ask for further advice from before you make any new claims to Pension credit if you have a younger partner who is below their pension age.

Charity for Civil Servants for you, by you

If you are a civil servant, past or present, or if you work for an organisation that is sponsored by a government department, you may be eligible to apply for help from the Charity of Civil Servants. They can advise on money issues, relationship difficulties, caring responsibilities, stress, anxiety, depression and other mental health problems. They can provide financial assistance where necessary and may be able to offer a one-off payment during a crisis.

City of Wolverhampton Council provides a Telecare service to support vulnerable people to live independently in their own homes and communities, and provides reassurance to individuals, carers and families 24/7.

Wolverhampton Telecare consists of a range of personal and environmental sensors that enable people to remain safe and independent in their own home for longer. It aims to provide reassurance 24 hours a day to those living on their own or caring for vulnerable people, helping you to stay safe and the right support in crisis situations.

Complaints Procedure

WHAT HAPPENS WHEN I COMPLAIN?

Stage One

We hope that most problems can be sorted out locally and informally. This should be done, if you feel able to, by first speaking to the member of staff who you have already had contact with. If the complaint is about that person, ask to see the person in charge. Tell them clearly that you are not satisfied and why.

Stage Two

How our Formal Complaints procedure works

If we have been unable to resolve your concernswith the person in charge or if you have asked for your complaint to beregistered as formal the following procedure will take place:

WHAT ACTION CAN I TAKE IF I AM NOT HAPPY WITH THE OUTCOME OF MY COMPLAINT?

If you are dissatisfied without final response, you should contact the Practice Manager in the first instance to discuss your concerns further. We will ensure that every effort is made to satisfactorily address any outstanding issues.

In the event that you believe the organizationhas not provided a satisfactory response to the matter that you confirmed inyour initial correspondence you can contact NHS England complaint service onthe contact details below.

They will pass your concerns to the local locality to be dealt with.
